2026-2027 Student Enrollment Information and Documents
Enrollment documents are fillable PDF documents. These documents may be downloaded to your home computer, then electronically manipulated to add information and print, or you may print the document and handwrite the information.
In-person enrollment is July 29, 2026, at 12:00-8:00 PM in the Udall High School Commons and Gymnasium located at the corner of Fourth Street and Seymour Street. Enrollment forms will be available for in-person enrollment.
Please complete the following documents per student to enroll in USD 463 for the 2026-2027 school year.

- USD 463 Student Medical Release Form
- USD 463 Immunization Authorization (NEW STUDENTS ONLY, i.e., transfer, new PreK, new kindergarten, enrolling to school for the first time)

- USD 463 3-year & 4-year-old Enrollment Form (Only for Preschool Enrollment)
